Concentration of stool by formol ether sedimentation can be done in asymptomatic or partially treated oocyst passers. A mucosal biopsy may be required sometimes for a definitive diagnosis. In direct immunofluorescence using polyclonal and mouse monoclonal antibodies, an apple green fluorescence is produced, having a sensitivity of 99% and a specificity of 100% for Cryptosporidium, Isospora, Cyclospora, microsporidia, and Giardia [Figure 2]. Diagnosis of Cyclospora oocysts may be problematic, as most laboratories fail to recognize them in direct fecal smears by modified Z-N stain [Figure 1]. In Safranin methylene blue stain.
