The anatomy of the orbital floor predisposes it to fracture. The inferior orbital neurovascular bundle (comprising the infraorbital nerve and artery) courses within the bony floor of the orbit; the roof of this infraorbital canal is only 0. 23mm thick, and the bone of the posterior medial orbital floor averages 0. 37 mm thick. By contrast, the bone of the lateral portion of the orbital floor averages 1.
Serious consequences of such injury include diplopia in upgaze where there is significant damage to the orbital floor. In blowout fractures, the medial wall is fractured indirectly. When an external force is applied to the orbital cavity from an object whose diameter is larger than that of the orbit, the orbital contents are retropulsed and compressed. The consequent sudden rise in intraorbital pressure is transmitted to the walls of the orbit, which ultimately leads to fractures of the thin medial wall and/or orbital floor.
